IMPORTANT: Hotel Reservations and/or Group Block Registration - Hilton Hawaiian Village
If you did not secure Hilton Hawaiian Village hotel reservations and/or register for the ADA Annual Session through Experient after receiving our February or March KEY-mail (email), you may do so now - online, by mail or by fax. Our block of rooms has limited availability from September 29 to October 3 - the largest numbers of rooms are available on October 1 and 2 - so book your hotel reservation soon using our code ICDHNL for both hard copy and online reservations (see website address below). We benefit from a reduced cost for many items if the ICD uses up its allocated room block. Our ICD block of rooms will be held until June 18, 2009 or until rooms are exhausted. Afterward, rooms will be assigned on a space-available basis at our hotel (Hilton Hawaiian Village) or nearby if space is no longer available. If you do not want to register online, please contact our office at 301-251-8861 for a hard-copy hotel reservation form.
A deposit of $250 will be due for each hotel room reservation, plus any registration fee for ADA activities, if applicable. Deposits and registration fees should be submitted online (credit card only), mailed (credit card or check) or faxed (credit card only) to Experient for processing. We benefit from a reduced cost for many items if the ICD uses its allocated room block; it is to our advantage for you to use our housing code for your reservations. See details below.
To register and/or reserve rooms in our block online, go to www.ada.org. Under "Announcements" click on 6th arrow, "Annual Session: Register Today", then click on "Complete Your Registration and Housing." Scroll down and click on "Group Housing Blocks." Here you can choose to "Register online for the ADA Annual Session" then type our Password, ICDHNL (all caps) in the box next to the red asterisk; OR print out a hard copy form by clicking on "PDF File/38k" - Write in our housing code ICDHNL in the top right corner of the printed form - Fax completed form to 800/521-6017 or mail to Experient, 568 Atrium Drive, Vernon Hills, IL 60061-1731.
IMPORTANT: If you plan to attend ONLY ICD functions, and do not wish to register for ADA events, follow these directions to avoid a registration fee: ONLINE: After typing in our Password, ICDHNL, click on "Continue". Scroll down to "Register Now" and click on "Start Here". On the "Record Lookup" page, SKIP 1) Dentist Record Lookup (for people attending ADA activities), and SCROLL DOWN TO 2) Dental Professionals and Constituent and Component Association Staff. Click on, "Click Here to Register". Fill out your Profile including the three questions below your email address, then click on "Next". Under Registration Type, choose Housing Only - No Badge Issued ($0 Registration fee). ON HARD COPY FORM, next to Advance Registration & Primary Registrant Codes and Fees, write in "Affiliate Housing Only - No Fee".
PLEASE NOTE: Registration and Housing - Changes or Cancellations
Hotel cancellations and/or changes must be received in writing by Experient before 5:00 pm Central Time on Thursday, September 3, 2009. Cancellations received by this date will receive a full refund of the hotel deposit by Experient. Please allow 60 days for refund processing. Failure to check-in to your hotel on the confirmed date of arrival will result in the forfeiture of the entire room deposit. As of Thursday, September 10, any changes to hotel reservations must be made directly with the hotel. Hotel deposit refunds will not be issued after September 3.
Advance registration for the ADA Annual Session closes at 5:00 pm Central Time on Thursday, September 3, 2009. Registration, ticket and/or special event cancellations must be received in writing on or before September 3, 2009 for a full refund, less a $10 processing fee per cancellation. No refunds will be given for registration cancellations after September 3, 2009 for any reason. Please allow 60 days for refund processing.
