The purpose of the International College of Dentists is to perpetuate the history of dentistry by recognizing those individuals, who have, by their actions, demonstrated conspicuous and meritorious service to the profession, their community, and their family. Any member, in good standing, may "Sponsor" a dentist within his or her local constituent (state) for Fellowship.

STEP #1 — MOST IMPORTANT ! !

Please submit the name of the candidate to the Deputy Regent for the Constituent. The Deputy Regent will submit the name to the screening committee for the purpose of determining if the candidate is a member in good standing in the American Dental Association and has been a member of the dental profession for a minimum of five years. Upon receiving a favorable response, the process may continue.

The fill-in Sponsor Form and the Candidate Form necessary for the fellowship process may be electronically downloaded as an Adobe PDF file from this website and may be completed on your computer screen, saved, emailed, and/or printed by using Adobe Acrobat Reader 8 (or later). Earlier versions of Adobe Acrobat Reader will NOT allow you to save or email the filled-in forms, only print them. The blank forms may also be printed out and then filled in by typewriter. Handwritten copies are not acceptable, except for signature.
