The purpose of the International College of Dentists is to perpetuate the history of dentistry by recognizing those individuals, who have, by their actions, demonstrated conspicuous and meritorious service to the profession, their community, and their family. Any member, in good standing, may "Sponsor" a dentist within his or her local constituent for Fellowship.

The fill-in "Forms" necessary for the fellowship process may be electronically downloaded as a Adobe PDF file from this website and may be completed on your computer screen, then printed out; or they may be printed out as blank forms and then filled in by typewriter. They are the Sponsor Form and the Candidate Form. Handwritten copies are not acceptable, except for signature. The blank forms can be downloaded, easily typed into on your computer, and then must be immediately printed ( They can not be saved with the information filled in ) so they can then be sent in via mail.

STEP #1 — MOST IMPORTANT ! !

Please submit the name of the candidate to the Deputy Regent for the Constituent. The Deputy Regent will submit the name to the screening committee for the purpose of determining if the candidate is a member in good standing in the American Dental Association and has been a member of the dental profession for a minimum of five years. Upon receiving a favorable response, the process may continue.
